Killing a baron

Will a new one be promoted in their place if they die or will you just be stuck at that one stage for ever?

No one is appointed, but you won't be stuck forever.  Keep going after the baron dies.  The dwarf monarch and entourage can still come.
 
My last fort went from Baron --> King.
 
My current fort went from Mayor --> Queen.
 
I think it has something to do with what the dwarf caravan reports back.  Strive to become a Metropolis, and you will get the monarch.
 
In the meantime, your fort will eventually elect a mayor so you can appoint a broker and such.

also if you strike adamantine before the monarch is going to arrive due to fortress growth. in a migration wave the monarch will show up in a disguise and randomly reveal themself.

Wait won't the barony be inherited if the baron has an heir within the fort?

Nope, inheritance after worldgen doesn't exist at all right now.  That should be changing in the next version.
