Mysterious Deaths of Metal Smiths

I was just wondering if anyone had an idea of why my dwarfs keep mysteriously dying at my magma forges?  As far as I know, they're built properly w/ no open spaces left unexposed.  I have no connections with any caverns in the area, but I did however get a notification telling me that a forgotten beast was seen from within one of the caverns.  I don't know if my dwarfs are dying from means other than work-related accidents, or is there some dangerous factor I'm not considering here?  The messages all say, "[name here] has been found dead."

There's a lot to consider and almost no causes are ruled out by your post, so be ready for some explanations that seem obviously wrong to you.

First, how many dwarves have you lost, were they all metal smiths, can you pinpoint where they actually died?

Check the combat logs.

Are the magma furnaces exposed to the magma sea or do you have it sealed off somehow. Imps and magma crabs have potentially deadly ranged attacks.

Can you find the FB on the map? Use 'u'. Does he have any projectile attacks: "beware his poisonous spit" or something.?
 I doubt it's the FB tbh but you never know.
Do you have access to the caverns? If your not absolutely sure, see if a squad can path to it. Station them: 's' 'your squad's letter' 'm'

I'll bet its something like a crundle, a berzerking dwarf / random tantrum, or cave swallow that somehow slipped your notice.
Do you 'd'ump things into the magma? I've lost dwarves somehow that way with the latest version.

Had this happen if the smoke from the burning items could reach the dwarves that were dumping.  Sometimes the smoke would stun them, make them panic or whatever but they'd fall in right after particularly if several other dwarves were dumping. I changed my dump setup to a 3zlvl fall to the lava and dwarves stoped takeing hot baths by mistake.

@yolatengo13> another question is are you runing a moded version of dwarf fortress?  I seem to recall one of the options in the Masterwork mod involved forge accidents along with other unique sorces of fun.  Dont know if those just injure the dwarf involved or if they can kill outright but thats another posibility aside from the options rude listed.

I changed my dump setup to a 3zlvl fall to the lava and dwarves stoped takeing hot baths by mistake.
This is nice to know, I figured it was either that, or the new physics collisions making dwarfs too close to the dumping dwarf dodge off into the red water.

Hit 'z' and look at the justice screen.  Did the dead dwarves violate some noble's mandate?  Something similar happened to me, and it turned out my captain of the guard was killing dwarves, I think because they "broke the law".

If you engrave a slab to memorialize them, you can then view the slab and it will tell you how they died.
