I'm getting people coming up in Dwarf Therapist who don't exist in game. Dwarves who aren't on my list in my fortress (not vampires that are mis-named in game, but more actual dwarves than there are). Also I keep getting a message in game about a Forgotten Beast that is no longer enraged, but that beast doesn't exist on any of the unit lists (enemy or otherwise).

the beast is in the caverns. Yes, it's weird to get that message, but it'll be there, better get used to it sorry.

As to therapist... Did you check the edges of your map? you might have gotten the trader bug. Basically, dwarfs migrate to your fort but due to a game mechanism don't join as migrants but show as visitors (other). TheRapist still shows them as migrants. DFhack has a fix for it

That particular FB finally appeared and started waging war with Amphibian men.

However, I located the existed of 2 of the dwarves in game that Therapist listed. They're ghosts.

Yes. They're ghosts. Ghosts in game. Dwarves in Therapist.
