hatch death?

I had a dwarf die.

He was at the bottom of my one ramp entrance.
He soon died.
There's a hatch on top of the ramp.
Could he have gotten on top of the hatch, and someone opened it beneath him?

Yes, I've also been getting tons of injuries due to hatches over ramps. Moral of the story is: don't put hatches over ramps.

This is an "acknowledged" bug that Toady is working on.

Bug #5925: "Bin" attacks "dwarf" while carrying it over a "ramp + hatch cover"

Any additional details you can add there might help.
