Why can't I mine out certain places?

It is worth noting I'm on a glacier, so I know stairs aren't supposed to work in places, but I can't get any designations to work on certain tiles.

I can mine some areas around where my dwarf is locked in its room, but not the majority of them.  There are a few regular rocks sticking out of the wall, and no success with them either.  I've tried every designation type, no luck.  Nothing works on the floor either.

He is the sole survivor of a zombie attack, and I can't have him relocate. He is slowly dying, but I was hoping to make it to the caverns and search around for a lake or something.

He does have a pick of course.

Any suggestions?

Damp stone maybe?

Or did you mess around with something like the liquid tool in dfhack? That can cause pathing problems.
