Question mark is a general 'confusion' icon - in my experience, it usually denotes that whatever the dwarf was about to do can no longer be done at this point in time.

! is variable - white, grey or purple ones indicate a strange mood (the good kind), while cyan, blue or red ones indicate insanity (babbling, melancholy and berserk respectively). Note that insanity cannot be cured. Well, in the sense that it can't be separated from the dwarf and have the dwarf survive.

A down arrow shows that the dwarf needs something - brown means it's hungry, blue means it's thirsty, grey means it's tired and needs sleep, and red means it's unhappy.

A + shows that the dwarf is injured in some way - yellow indicates minor wounds or non-bleeding wounds, while red means the dwarf is heavily wounded and/or losing blood. Lost limbs manifest as a yellow + when they get treated.

Well it seems as though my dwarf's are dying of Thirst. How do I counteract this? All of the rivers are frozen and I have run out of Supplies.

Probably too late now, if you have no plants left. If you do have plants, build a still and make booze.
Next time, there's two things to do - dig out a pit and fill it with water (either by bucket brigades or by tapping into flowing water), and farming plants so you have a source of booze (everything except dimple caps can be brewed).

Farming is relatively easy, you need either soil/sand/clay, or wet stone (read below) and some seeds. Underground crops grow underground, you can also collect surface crops when you arrive with Herbalism to farm above ground.

Bucket brigades can be set up like this: 1, make several buckets from wood. 2, dig a pit. 3, hit "i" and designate the edges of the pit as a Pond - multiple small Pond designations around the perimeter work better, up to one per bucket/dwarf. 4, wait for the dwarves to fill it. 1/7 water evaporates, so you'll want to fill it higher, up to 7/7 and periodically refill it as it gets used.
Now, to wetten stone, you'll need a hole a level above the place you want a farm plot, and dump water through it until you have a nice puddle of 1/7 water, whicl will evaporate and leave mud that you can farm on. This works on constructed floors too.

Thanks for the info! Although it was an inevitable demise. One dwarf eventually went insane and berserk killing all. Puppies chicks and people, anyone he could. They tried to fight back but he had a pick so they did not fair well. We will see next game.

Congratulations! Your fort was a critical success. Next time you might want to pick a Warm biome so the water doesn't freeze.

What kind of brew does one get from quarry bushes? :P
